Client Intake Forms

Before arriving for your first appointment ...

You will want to complete the appropriate forms on this page before coming to your first appointment. Some of the documents do not need to be printed – noted “review” only. All clients need to review the “Office Policy and Confidentiality” form along with printing out the “Important Signatures” document. For clients receiving marriage counseling, you do not need to complete the individual counseling forms. However, both spouses need to complete ALL of the forms listed under "Marriage Counseling Forms" section.
